Lauren Ziaks, PT, DPT, ATC, NCS is available to provide telehealth physical therapy services for patients living in PTCompact States (learn more here). She also is fully licensed in Idaho and New York.
Lauren specializes in the care of patients with concussion, chronic dizziness, long-COVID (post-viral infection syndromes), and other causes of autonomic spectrum disorders (dysautonomia).
The Initial Evaluation
Once you establish care with Doctor of Physical Therapy, Lauren Ziaks, you will complete in-take paperwork to provide a thorough history including your past medical history, how your symptoms started, timeline of previous treatments, and any other important information to provide you with the best care possible.
If autonomic dysfunction (dysautonomia) is suspected on initial screening, you may be asked to obtain specific equipment prior to our first session.
Your evaluation is expected to take 60 minutes with follow up information provided after all visits via email.
After a thorough evaluation, we will work together to create the best treatment plan for you.
Care is provided with our evidence-based protocols however, each plan is tailored to the person on the other side of the screen.
Follow-up Appointments
Allow 55-minutes for each follow-up session to ensure we have time to answer questions and progress your individualized home exercise program.
Before each session you will complete an online symptom questionnaire to track your symptoms and progress.
Patients typically can expect to be seen 1 time per 2-4 weeks.
Your plan of care can change at any time throughout the process based on how you are doing and how your needs change over time.
Home Exercise Program Expectations
Each patient is expected to complete 20+ minutes per day of home exercises in order to achieve their maximal healing potential.
You read that right, “Nerves that Fire Together, Wire Together.”
This treatment plan will not work for you if you aren’t willing to work for it!
Equipment Needs
Information about materials and equipment needed for your initial evaluation and throughout your care will be provided.
Each required item has been thoughtfully selected to limit out of pocket costs for patients while maximizing healing potential.
As we will meet over telehealth, these materials will be required to extend the treatment space into the comfort of your home or local gym.
Primary examples: chest strap heart rate monitor, exercises to print at home, access intermittently to a treadmill for patients with dysautonomia.
Where is Treatment Available?
Lauren is currently licensed / holds privileges in: Utah (home state), Washington, Oregon, Idaho, New York, and Colorado.
If you live outside of these states (and not in a PTCompact State), you can discuss options with Lauren for licensure in your state.
Due to different state requirements, a license in your state may or may not be feasible.
If a licensure in your state is not possible, PTCompact States are expanding in number and it may be an option for you in the near future.
It is required for Lauren to be fully insured and licensed in both the state you are located and where she resides. If you will not be located in a state meeting the above requirements at the time of your appointment, we will have to reschedule.
*Lauren is currently not accepting patients in Utah. If you would like to be seen in her home state, information can be provided upon request.*
Housekeeping - how, where, cost
All visits are fee-for-service (cash) however, a “super bill” can be provided for you to submit to your insurance independently.
Initial Evaluations & Follow-ups: $225
Package discounts available for groups of 4 sessions: $200 per session
Your sessions will be performed via telehealth on a secured platform, instructions will be provided before we get started.
A computer or tablet is preferred, but sessions can be completed via cellphone.
Expect to move during your PT sessions, an environment where you can perform movements safely is best.
Your documentation (treatment notes) will be stored in accordance with your state’s laws.
Doctor Referral?
Most states allow for open access in Physical Therapy. This means you do not need to have a referral to start care.
A doctor, nurse practitioner, or physician’s assistant referral may required after the first 30 days of treatment depending on your location and state laws.
You can discuss your state’s requirements with Lauren during your initial evaluation to set you up for success.
It doesn’t matter if your injury was 3 days or 10 years ago, patients who work hard in this model WILL see improvement.

Lauren Ziaks, PT, DPT, ATC, NCS has over 8 years of experience treating individuals with concussion and has helped >1200 patients improve their quality of life. Through this growing and evolving practice, she has played a key role in the development of new treatment protocols to help close the intervention gap in knowledge that exists in concussion management, long-COVID, and autonomic disorders.
She is a graduate of Northeastern University with both a Bachelor of Athletic Training and a Doctor of Physical Therapy.
She was previously certified in vestibular rehabilitation therapy with the American Institute of Balance but earned her vestibular competency through Duke University in 2021. She is a board-certified Neurologic Certified Specialist (NCS). Lauren has advanced training in vision therapy and operates her practice under the direction of a FCOVD neuro-optometrist. These advanced trainings have allowed her to develop integrated vision and vestibular therapy protocols – treating the brain the way the brain is used in real life vs in specialty silos.
Following her own traumatic brain injury and subsequent POTS diagnosis, Lauren has dedicated her practice to helping others Return to Life and Sport. She created Phoenix Concussion Recovery to provide an educational resource to patients and providers alike.
Lauren has published her novel concussion evaluation and treatment protocols in peer-reviewed journals and is now implementing these protocols with success for patients with long-COVID.
Over the last 4 years, her practice has expanded into the management of autonomic dysfunction (dysautonomia) and in addition to new publications, she played a key role in the creation of the Utah ADaPT protocol.
She presents at local and distant institutions, local, state, and national conferences and her aim is to train as many providers as possible in her novel techniques to improve patient outcomes across the United States.
